What is Creative Care Options?
A Care Management Organization (CMO) providing long-term care services for
elderly and disabled individuals in Fond du Lac County. It is a partnership
between Fond du Lac County and the state of Wisconsin. Consumers of services
are involved in the on-going planning, implementation, evaluation and
improvement of services.
You can become a member of Creative Care
Options if:
- You are 18 years or older
- You live in Fond du Lac County
- You need support and assistance with care
- You meet the financial eligibility
- You sign the Enrollment agreement

Types of Services:
- Personal Care - assisting with bathing,
dressing, grooming, meal preparation, etc.
- Chore Service - grocery shopping, cleaning
- Durable Medical Equipment & Supplies -
syringes, wheel chairs, crutches
- Transportation - rides to medical
appointments, work, recreational activities
- Home Health Services - nursing services,
Hospice
- Therapy Services - physical, speech and
occupations, etc.
- Assistive Devices - hearing aids
- Prevention and Wellness Education - group
seminars, individual counseling
- Skills Training - daily living skills,
independent living
- Service Coordination - team consisting of a
nurse, social worker, and others
