Referral
Process: once
the paperwork is submitted in Ukraine, the Ukrainian Center of Adoption reviews
it. The Center of Adoption will set-up an appointment with the parent(s) and
issue an approval to travel to the orphanage to select a child. The parent(s)
will be escorted to the orphanage and assisted during the selection process. No medical
information about the child will be available before travel. Parent(s) must
contact a US pediatrician before they travel and keep lines of communication
open during their trip. Our office can act as a liaison between the parent(s)
and the doctor. A local doctor will be contacted to evaluate the child in
Ukraine. A list of physicians will be available for the adoptive parents and one
can be selected during the trip. International
Travel is mandatory: The
adoptive parent(s) have to be prepared to spend 3 to 4 weeks in Ukraine
(to finalize the adoption) and Poland (to obtain the child’s visa).
Parent(s) spend most of the time with the child. An escort will be provided
during the stay in Ukraine. We are in the process of locating escorts for the
trip to Poland, escorts that will meet the parents at their port of arrival, and
escort them to the hotel, make necessary reservation and accompany the parents
to the US Embassy and airport as well. Child’s
Visa: is issued at the US
Embassy in Warsaw, Poland. Foster
care is not available for the
Ukrainian program. Paperwork: Following is the paperwork included in the adoption dossier. Ø Form I-600A - approved by regional Immigration and Naturalization Office. Ø Form I-600 - will be filled at the US Embassy in Warsaw, Poland. Ø Proof of citizenship - Birth certificate or Certificate of citizenship, copy of US Passport. Ø Proof of Marriage and Prior Divorce(s), if any. Ø Home Study. Ø Police Record – issued by the local Police Department. Ø Commitment to Ukraine Center of Adoption – to register and provide periodical information to the consular office at the Ukraine Embassy in the USA. Ø Power of Attorney – for the Ukrainian attorney. Ø Medical Record – a letter from your family physician stating your physical and mental health condition. Ø Approval Letter – from the Department of Health and Social Services. |
