The details of the Japanese accident at the Uranium factory were pretty unbelievable. They had workers not following procedures, not using proper equipment and not wearing dosimeters. They actually poured a slurry of uranium dissolved in nitric acid that was 4 times critical mass in a metal bowl and were stirring it with a wooden spoon. Well, they realized there was a problem when people starting seeing blue flashes. (Chernekov radiation from the water in their eyeballs. High speed particles exceeding the speed of light in water appear blue.) Geez, anyone who completed high school knows that mixing 10 pounds of enriched uranium in a bowl would be a bad idea. Procedure said that they were not to have more than 1 pound in a single container. So they had a mini reactor running in a mixing bowl that ran for more than 20 hours. Gross incompetence at many levels.
