MPAG Forms and Document Use

 

MPAG Membership Liability Form: Must be signed by all MPAG members, and especially, for minors.

Member Information questionnaire form: Should be filled out for all members.

Public Access User Compliance Agreement Form: Must be filled out by anyone who submits a program for submission, live or taped. It isn't required for every tape submitted by the same person or group, as long as they have such a form on file. But for most individual, non-series, submissions by the public, and for any non-MPAG production (other than series), it is a very good idea to have one filled out for each new show.

Videotape Submission Form: Required for each taped program submitted for airing. A double-sided form, the front must be filled out with all relevant information, and the back side is a legal agreement which must be filled out and signed.

Old tapes will eventually have one of these on file for them, as well (those before 1996, most do have this). But all new tapes should (100%) have this form prepared before airing is allowed.

Playback/Program Scheduling Request Form: This is used to ask for specific times for a given program to play, or to add other information about when a program should be on. It isn't needed, if you are willing to let MPAG do the scheduling for your show.

Program/Production Planning Request Form: You use this to help figure out what equipment will be needed, or how many people, and when, for a given program. It should be available to members and the public.

"Special Event" Application Form: This is used to request MPAG to volunteer to tape or air live programs.

The Tape Copy Request Form: used for purchased tape copies.

Other Documents

MPAG Guidelines: Our rulebook. Should have a copy for each member. Should be at least a couple copies around for public to look at, one in cable room at all times. Should be able to give out copies to members of public who ask for it, with charge to non-members for copying costs.

Guidelines for Taping/Airing Public Events for Public Access Broadcast by the Muskego Public Access Group (Muskego Access Channel 7):

This needs to go out to every group that will use the "Special Event" form, as it is mentioned on it.

Part of the MPAG Guidelines, "Guidelines for Taping/Airing", sections F and G of the guidelines, are also kept as a separate document for attaching to this, as it goes along with these.

Copies of this document should also be given to MPAG members, without the attached MPAG guidelines section (since they get a full copy).

Guidelines for Tape Submission for Airing on Public Access TV:

This should be available to any member of the public who wishes to submit a tape for airing, and should go out to any organization which is expected to tape its own events or produce its own programming for submission.

Part of the MPAG Guidelines, Section G (and G-A), should also be available along with this document.

All MPAG members should have a copy of this.

Suggestions for Airing Playback Scheduling Requests: Should be available to anyone who will use the Playback Scheduling Request form; should be available with both of the above two guidelines.

 

MPAG Member Packet: Should include one (1) of all of the above documents. Attachments of partial MPAG Guidelines sections are not needed, as a full copy of the MPAG Guidelines will be included.

Page count, estimates: MPAG Guidelines, as formatted, 16 pages (8 double-sided). Sections F/G, for "Special Event" requesters, are 8 pages (4 double-sided) as formatted, but we could probably compact it down to 2 or 3 double-sided pages. Section G, the Airing Operations section, is about 6 pages (3 double-sided), but we could make it about 2 pages double-sided; making section F 1 page, G two pages, for attachments. We plan to keep the larger type for the MPAG member books, for readability.

Both Guidelines for "Special Event" and for "Tape Submissions" are four pages, 2 double-sided. The "Suggestions for Airing Playback Scheduling Requests" is only one page, and shouldn't go beyond that even if we go to double-sided. All forms are one page, with the Videotape Submission Form being double-sided.
